Using the Box upload portal below, you can securely submit documents containing protected health information or other sensitive information to Portland DBT Institute. Before uploading, please make sure that all file titles include the client’s full name and/or a brief description of the contents so we can route your documents more effectively! We accept most standard document and image file types, but please convert HEIC/HEIF files to a standard image file type like JPEG/JPG before uploading.
